You're right, it's definitely not the first time someone ever came up with the idea of genderswapping Draco. Even redteneri said that their own initial genderswapped Malfoy drawing was "kinda inspired" by this very one by grizz.
The wonder and awe isn't about it being the first ever draco_malfoy genderswap image (which undoubtedly came into existence shortly after the first book was released, long before little Tommy Felton ever slicked his hair and put on the green and silver). It's about seeing the ultimate inspiration of the current viral trend.
grizz literally renamed his fem Draco "Malfoid", so there's no point changing any random "Draco Malfoy genderswap (mtf)" into "Malfoid" unless it happens to them too
