When prepared correctly, fresh fugu still contains very little, but nonlethal-amount of tetrodotoxin, which, depending on individual sensitivity, may or may not cause the telltale 'tingling'; most caught fugu has been 'fasted' for few days to reduce the amount of tetrodotoxin remaining in their system (since tetrodotoxin comes from their diet, and farmed fugu contains literally zero tetrodotoxin) But people (being idiot as always) wanted that 'tingly' feeling, so (second rate restaurants) started putting tetrodotoxin into their farmed fugu dishes to make those thrill seekers' happy. The extraction method itself's very easy, and can performed without sophisticated equipment ;just dry either the ovary/liver and ground them into powder, then soak the powder with alcohol for some time to let the tetrodotoxin leech out. Filter the resulting mixture, and let the remaining liquid evaporate, resulting into white-ish powder. Then you test the powder's potency, and use them. If you want to get a more concentrated toxin, use fresh intestines. This 'method' is most certainly illegal, however it is said you can find these restaurants in back-allies of almost all fishing district.

As for trivia: since tetrodotoxin also present in the fugu's skin, fugu fin sake is basically drinking tetrodotoxin laced sake, albeit in very small doses since the fin itself needs to be processed by drying and charring to reduce the amount of toxin (tetrodotoxin is resistant against heat) and create the smoky flavour.
