I think it's because of the way people portray their personalities and determine them by bust size. For example, giving an idiot big breasts and making the serious one a flat chest. It's a strange world we live in, my friend.
I believe the roots of that largely lie in Sanae's outside world origins and divine lineage. One popular notion is that Sanae's that way due to being raised on modern japanese diet (more fast food/beef than say very traditional gensokyo).
While it's not the first topless one, it's one where he tried for a cute expression instead of one of his usual faces. And it's a pretty good attempt.

I doubt it's popular, I have yet to see anyone in the fandom say this. It's probably not that either given that by that logic the actual population of Japan would have way larger breasts.

probably because flat chests are much closer to reality and quote on quote smart anime girls are just closer to real women, which aren't quite as dumb as often in anime, you know what i mean?
